Those that delivered at Abington (or about to)

I'm filling out my preadmission paperwork and there's a yellow form that says "Maternity Reservation (Not Elective C-Section)"

There are no instructions that tell me what this form is for and if I need to fill it out (I am not having an elective c-section).  The main preadmission registration form already asks about my due date so I'm not sure why I have to fill out a similar form.

So do I need to fill it out?  There is a spot at the top that says "Reservation Date".  Should I fill out my EDD in that spot?  And what does EDC stand for?

Yes, I did call them and left a message but I kind of want to get all this paperwork done so I can mail it today.

  • I know exactly what you are talking about. I didn't fill it out and I didn't have any problems. They had all of my information. Don't send it in.

  • oh and also, don't send in the anestesia form. They will have one there for you the day of if you decide to go the epi route.

  • Thanks HW.  I actually have to have a consult with the anesthiologist before my delivery so I planned on taking the paperwork to them because I'm not clear on all of it.

  • I did not get any of these forms. Did you get these when you went for a tour of the hospital or something? We have to still set that up...
  • No, my OB gave them to me at my first prenatal appointment.
